Shoulder Pain Every Time You Reach? Stem Cell Treatment, Honestly

Reaching for a seatbelt, pouring a kettle, sleeping on your side — all suddenly painful. When physio and steroid injections stop working, stem cells enter the conversation. Here is the honest picture.

Medically reviewed by the Bangkok Stem Cell Center medical team

It usually starts small: a twinge reaching into the back seat of the car, a ache after gardening. Then one night you realise you cannot lie on that side, and getting dressed has become a careful operation. The scan says "rotator cuff tendinopathy" or "partial tear". Physiotherapy helped for a while. The steroid injection helped for six weeks. Now someone mentions stem cells.

What is actually wrong in a painful shoulder

Most persistent shoulder pain involves the rotator cuff — four tendons that stabilise the joint. Tendons have poor blood supply, so once they degenerate or tear, they heal badly. This is exactly the kind of problem regenerative medicine targets: tissue that wants to heal but cannot quite manage it. It is also why the shoulder has become one of the most common targets for commercial stem cell and PRP injections.

What the shoulder stem cell studies show

  • A frequently cited French study followed patients who received bone-marrow concentrate at the time of rotator cuff surgery; the cell-treated group showed better healing on scans and fewer re-tears over ten years. Promising — but the cells were an add-on to surgery, not a stand-alone injection.
  • A 2017 systematic review of bone marrow cell concentrate for tendon problems, including the rotator cuff, found the human evidence consisted of small, low-quality studies — enough to justify more research, not enough to recommend routine treatment.
  • For stand-alone injections into degenerated tendons without surgery, good-quality randomised evidence remains thin.
  • No study shows that an injected cell product reliably regrows a torn tendon back to normal.

What to try before considering injections

  • A properly progressed exercise programme from a physiotherapist — still the treatment with the best evidence for rotator cuff pain, and often under-dosed or abandoned too early.
  • Load management: modifying, not avoiding, the activities that flare it.
  • Treating the whole picture: sleep position, diabetes control (which strongly affects tendon healing), and smoking.
  • A surgical opinion if there is a full-thickness tear with weakness — some tears genuinely do better repaired.

If you do consider a stem cell or PRP injection

  • Ask what exactly is being injected, where the cells come from, and how they are processed and tested.
  • Ask whether the injection is ultrasound-guided into the specific damaged structure.
  • Ask which published study supports this protocol for your specific diagnosis.
  • Ask what the rehabilitation plan is afterwards — an injection without rehab is rarely the answer.
  • Be wary of packages that combine the injection with hard selling on price and urgency.

The honest bottom line on stem cells for shoulder pain

The shoulder is a plausible target for regenerative medicine, and the surgical add-on data is genuinely interesting. But for the typical person with a painful, degenerated rotator cuff, the evidence for stand-alone stem cell injections is still weak — while good physiotherapy, done properly and long enough, is strong. Exhaust the proven options first; they work more often than people expect.

Frequently asked questions

Short, plain answers to the questions patients ask most — so you can read with clear expectations, not promises.

Can stem cells heal a torn rotator cuff?

Not reliably as a stand-alone injection. The best data comes from cells added during surgical repair, which improved healing rates — a different situation from injecting an unoperated tear.

Is PRP the same as stem cell therapy?

No. PRP is concentrated platelets from your own blood; stem cell products contain cells from bone marrow or fat. The evidence for both in shoulder tendons is still limited, though PRP has been studied more.

How long should I try physiotherapy first?

Rotator cuff rehabilitation typically needs 12 weeks or more of properly progressed exercise before it is fair to say it has failed. Many people have never had a truly adequate programme.

When is surgery the right answer?

Full-thickness tears with significant weakness, traumatic tears in active people, and tears that keep enlarging are situations where a surgical opinion is worthwhile.

Are shoulder stem cell injections safe?

Injections into the shoulder are generally low-risk when done with sterile technique and imaging guidance, but infection and flare-ups occur. The bigger risk is paying for a treatment with weak evidence while delaying care that works.

Does diabetes affect shoulder healing?

Yes — diabetes significantly impairs tendon healing and increases the risk of frozen shoulder. Good blood sugar control is a genuine part of shoulder treatment.
